Repeat prescription information

If you are taking medication regularly long-term then the doctor may decide that you do not need to see him/her each time you need a new supply of medicines. The doctor will add the medicine you need regularly to your record as a repeat drug, which you can order when your medicine is running low.

Medicines which you are allowed to order as a repeat are listed on the other half of the prescription form. This is known as the ‘repeat slip’.

You can order a repeat prescription either by post, fax, online or in person. To order a repeat prescription by post, fill in the repeat slip or write the details on a piece of paper and post it to the following address:

St James’s Surgery or Junction Road Surgery
Northampton Buildings 8 Junction Road
Bath BA1 2SR Oldfield Park
Fax 01225 428398   Bath BA2 3NQ
Fax 01225 480045

To order a repeat prescription in person fill in the repeat slip and hand it in to reception. If you do not have a spare repeat slip, a request form can be obtained and filled in at reception.

Prescriptions can be collected from Reception or posted to you (provided you supply a SAE). Alternatively, you may choose to have your prescription sent directly to a pharmacy, by selecting the appropriate pharmacy from the list on the repeat slip. We can send your prescription to the following pharmacies:

Boots (Southgate)
Hawes Whiston (St James’s Square)
Hounsell & Greene (Oldfield Park)
John Preddy (Moorland Rd)
Lifestyle (New Bond St)
Luther Wilson (Brock Street & Newbridge)
Moss (Weston High Street)
Superdrug (Westgate Street)
Wellsway (Hounsell & Greene)

If ordering dressings, catheters or ostomy bags then it is useful to the receptionist if you give the code/reference number on the form.

Collection Times
If the prescription is ordered on the official repeat slip that is on the right hand side of your prescription, and provided there is no query over you medication, your prescription will be ready for collection within 2 working days (Monday to Friday); otherwise it will take up to 3 working days before it can be collected.

If an item is requested that is not on the repeat list then the doctor may wish to see you before it is issued.

After receiving items on repeat prescription for a length of time the doctor may request that you make an appointment to attend the surgery for a medication review with either a doctor or a nurse.

Medication reviews are important to ensure that your medicines and doses are appropriate and that you are not experiencing any unwanted side effects. It is important that you attend review appointments even if you feel well and are happy with your medicines in order for the doctor to monitor your progress.

If you are asked to attend the surgery for a review then no further prescriptions will be issued until you have attended the review appointment.

Please do not use urgent appointments for medication review.
